2013 Examination Dates

The following must be received in the division by the examination deadline date:

Complete notarized application

Supporting documents of training (supporting documents must be original or certified true copies, i.e., student monthly reports or certification from another state regarding hours).

A check or money order payable to State of Alaska for the following:

Application Fee (nonrefundable) of $80.00

Examination Fee of $95.00 ($35.00 for written, $60.00 for practical)
