Name/Address Change

It is your responsibility to notify this office of name/address changes and to keep this information on record current.

There are three sections to this division (see below), each with its own filing and reporting requirements based on the specific statutes (law) and regulations for each program within each section of this division.

Professional Licensing Section

To notify professional licensing of a name change and/or address change, submit a Name Change and/or Address Change form (#08-4291).

Corporations Section

There are multiple addresses on an entity’s record and changing each one requires a different form. There are different versions of most forms based on entity type. Select the correct form based on your entity type at Corporations Forms By ENTITY Type .

Registered Agent Name or Addresses Change:

Per statutes, to update or change the registered agent information (name or addresses) submit the appropriate Statement of Change form along with its $25 filing fee. NOTE: There is a different form for each entity type, make sure you submit the correct version based on your entity type.

Entity Addresses Change:

A change to an entity name requires an amendment. Submit the appropriate form along with its filing fee. NOTE: There is a different form for each entity type, make sure you submit the correct version based on your entity type.

Officials Names and/or Addresses Changes:
  • If the Biennial Report is due then update the officials’ names, addresses, and other statutorily required information on the Biennial Report. Go to the Corporations Section and click Biennial Report.
  • In-between biennial reports, submit the appropriate Notice of Change form along with its $25 filing fee. NOTE: There is a different form for each entity type, make sure you submit the correct version based on your entity type.
Duplicate Entity certificates and documents:

Business Licensing Section

Change in business name requires a new business license.

Go to the Business Licensing Section and click New Alaska Business License.

Change in Owner requires a new business license.

Go to the Business Licensing Section and click New Alaska Business License.

Change in owner’s legal name (i.e. maiden name to married name)

Submit Business License: Owner’s Legal Name Change (form 08-4104) along with the appropriate fees.
